At Amazing Smiles, we work with many different insurance companies on behalf of our patients. We pride ourselves on handling all insurance claims for each patient to make the experience efficient and hassle free. Let us do all the work for you when submitting your claim. Our staff is prepared to answer all of your questions regarding your eligibility and coverage.
Please bring your insurance card to your appointment. If you do not have a physical card, bring a printed or digital photo of the front and back of the card. There is information on both sides of the card that we need in order to properly enter your insurance and avoid future issues with claim payments. For those patients that are insured through a spouse or family member, we will need the policy holder's name, date of birth and employer as well. Please be prepared with all of this information the day of your appointment. All patients are expected to pay their portion not covered by insurance at the date of service.
